Assigned Forms

The Personal Planner > Lists tab > Assigned Forms sub-tab allows you to view and manage forms assigned to you.

To manage assigned forms:

  1. Open the Personal Planner to the Lists tab and click the Assigned Forms sub-tab.
  2. Select the checkboxes as necessary:
    • Include Group Assignments: Includes forms that are assigned to a group that you are a member of in the list.
    • Include User’s Previous Assignments: Includes forms that are still open and were previously assigned to you.
    • Show Open Forms Only: Only display forms that are still open/active and not yet complete.
    • Note: If you have access, you can edit the User and Provider fields to view forms assigned to a different provider.

  3. Click the Search for Data () icon to display your list of assigned forms.
  4. Review the listed entries.
  5. To perform another task, complete any of the following:
