The Personal Planner > Lists tab > Assigned Forms sub-tab allows you to view and manage forms assigned to you.
To manage assigned forms:
- Open the Personal Planner to the Lists tab and click the Assigned Forms sub-tab.
- Select the checkboxes as necessary:
- Include Group Assignments: Includes forms that are assigned to a group that you are a member of in the list.
- Include User’s Previous Assignments: Includes forms that are still open and were previously assigned to you.
- Show Open Forms Only: Only display forms that are still open/active and not yet complete.
Note: If you have access, you can edit the User and Provider fields to view forms assigned to a different provider.
- Click the Search for Data (
) icon to display your list of assigned forms.
- Review the listed entries.
- To perform another task, complete any of the following:
Add Forms
- Click the Create a new Record (
) icon to display the Select Patient window.
- Search for the patient and double-click the record to display the Add Form window.

Complete the fields as necessary and click OK.
Edit Forms
- Select a listed record and click the Edit Record (
) icon to display the Forms window.
- Modify the fields as necessary and click OK.
Edit Form Information
- Select a listed record.

Right-click and select Edit Form Information to display the Forms window.
- Modify the fields as necessary and click OK.
View Form Events
- Select a listed record.

Right-click and select View Form Events to display the Form History window.
Re-assign Forms
- Select a listed record.

Right-click and select Re-assign Forms.

Click each ellipsis to select a Messenger Group and/or User and click OK.
Select Patients
- Select a listed record.
- Right-click and choose Select Patient.
